Three things that make or break fit...
Shoulders first always.
The seam must sit at the end of your shoulder. This is the one measurement that can't be tailored. Get it right before anything else.

Chest without tension.
Button up with arms at your sides; no "X" of pulling at the button. If it pulls, go up a size and tailor the waist instead.
Sleeves are easy to fix.
You should see half an inch of shirt cuff below the sleeve. That's a quick tailor job! Never reject a blazer over sleeve length alone.
